Traditional concepts do not map one-to-one onto modern diagnoses. Read the divergence note first.

The traditional concept is considerably broader. A 2020 conceptual review concludes that the core of this entity is not mood but a mechanism of stagnation, with chest oppression and a lump sensation in the throat as characteristic features — pulling in somatic symptoms that ICD-11 places outside its mental and behavioural chapter. The WHO standard terminology likewise defines it as a general term rather than a single disorder. Conversely it does not require the defining core of a depressive disorder — depressed mood with loss of interest or pleasure — and carries no duration threshold (ICD-11 requires two weeks), no severity grading, no episode count, no functional-impairment requirement, and no suicide item. It also does not separate unipolar from bipolar depression. Note — Two papers in the same journal disagree: a 2011 review treats it as the Korean medicine counterpart of depression, while the 2020 conceptual study concludes it is distinct from depressive disorder.
