The traditional concept is broader: it is defined by a single phenotype — flaccid weakness of the limbs — irrespective of where the lesion lies. The six conditions the research identified already cross four ICD-11 chapters and four distinct lesion sites: neuromuscular junction (myasthenia gravis), anterior horn cell and corticospinal tract (amyotrophic lateral sclerosis), nerve root and peripheral nerve (Guillain-Barré), and muscle itself (muscular dystrophy, polymyositis). Conversely it captures none of the distinctions that separate those six in treatment and prognosis: fatigable versus fixed weakness, upper versus lower motor neuron signs, rate of progression (days for Guillain-Barré, years for dystrophy), or antibody, creatine kinase and demyelination findings.

Signs that need immediate medical assessment

Weakness that progresses rapidly over days — particularly ascending weakness, bulbar symptoms (dysphagia, dysarthria) or breathing difficulty — may be Guillain-Barré syndrome or a myasthenic crisis. It calls for emergency neurological assessment where respiratory function can be monitored, with consideration of intensive care admission, not outpatient conservative treatment.
