The traditional concept is broader: it groups generalised pain, cold intolerance and paraesthesia appearing after childbirth under one name, and the research places chronic widespread pain and chronic primary musculoskeletal pain within its range. Conversely it carries none of the axes that separate them. The modern chronic pain categories require duration and functional impairment, and code separately for causes confirmable by testing, such as postpartum thyroiditis or rheumatic disease. That differential forms no part of the traditional diagnosis.

Signs that need immediate medical assessment

The Korean medicine clinical practice guideline for postpartum wind syndrome (Society of Korean Medicine Obstetrics and Gynecology, 2024) defines it as a diagnosis of exclusion and specifies that hyperhidrosis, postpartum depression, hypothyroidism, rheumatoid arthritis, hypopituitarism and flaccid weakness must be ruled out before the diagnosis is made. In particular: (1) hypothyroidism and postpartum thyroiditis present together with pain, fatigue, oedema and cold intolerance, making them hard to distinguish, and a family history raises the incidence of postpartum thyroiditis tenfold, so thyroid function blood tests are needed to exclude them (ICD-11 JB44.5 Postpartum thyroiditis: transient hyperthyroidism and/or hypothyroidism in the puerperium). (2) The guideline recommends the Edinburgh Postnatal Depression Scale for screening postpartum depression. (3) Hypopituitarism following major postpartum haemorrhage (Sheehan syndrome) is also to be excluded. The guideline states that blood tests and imaging should be referred for where necessary. In addition, before postpartum pain is explained as this syndrome, the CDC HEAR HER and AIM/ACOG Urgent Maternal Warning Signs (applying for up to one year after birth) require immediate care for: fever of 38.0°C (100.4°F) or higher, severe abdominal pain that does not go away, heavy vaginal bleeding or foul-smelling discharge, chest pain, a racing heart or difficulty breathing, severe headache that does not go away, swelling, redness or pain in a leg (suggesting deep vein thrombosis), and thoughts of harming oneself or the baby.
